In this podcast, Dr Heidi Moawad interviews Dr Cesar Ochoa-Lubinoff, MD, MPH, FAAP, Associate Professor and Division Chief of developmental-behavioral pediatrics at the Rush University Medical Center.

Angelman syndrome (AS) is a rare, genetic neurodevelopmental disorder. Patients with AS often have profound developmental delays, movement and sleep disorders, seizures, and severe speech impairments. This condition is caused by a loss of function of the imprinted UBE3A gene on maternally inherited chromosome 15. There are no FDA-approved treatments for AS, with current therapeutic approaches focusing on managing disease-specific symptoms. In this podcast, Heidi Moawad, MD interviews Cesar Ochoa-Lubinoff, MD, MPH, FAAP, about recent encouraging clinical trial results in patients with AS.

OV101 is an investigational medicine being developed by Ovid Therapeutics that is designed to target impaired tonic inhibition, a core mechanism of the AS neuropathology. The continued study of OV101 is a critical step forward for the AS community as it is the first targeted brain treatment for AS.

Dr Ochoa-Lubinoff is a board-certified developmental-behavioral pediatrician, Associate Professor and Division Chief of developmental-behavioral pediatrics at the 

, and Medical Director of the Rush Neurobehavioral Center, Chicago, IL. He is the Co-Director of the Rush Angelman Syndrome clinic sponsored by the 

Dr Ochoa-Lubinoff has established a multidisciplinary developmental-behavioral pediatrics clinical program at Rush that evaluates and provides specialized follow-up care including psychopharmacological management to children and adolescents with AS, Fragile X syndrome, autism spectrum disorders, Down syndrome, and other complex neurodevelopmental conditions. He has been an investigator in clinical trials for AS, Down syndrome, Fragile X, autism, and other neurodevelopmental disorders. He is the Rush principal investigator for the Angelman Syndrome Natural History study.

Dr Ochoa-Lubinoff reports that he is a consultant with Ovid Therapeutics.

 Thank you for this rich and informative discussion, before we conclude I would like to get final thoughts from each of you.

 When we talk about a rare disease like SMA [spinal muscular atrophy], the most important point is early diagnosis. This makes all the difference in trying to change the natural history of the disease; every second counts. If you make an early diagnosis, you have a chance to start treatment early. Another point to emphasize is the need for teamwork. The multidisciplinary team needs to communicate with transparency so that they understand patient needs and can make the appropriate clinical follow-up. Finally, we need to empower family members, parents, and caregivers to join patient associations, and try to shed light onto these diseases. Patients are out of sight because no one thinks about the possibility of a rare disease. It is critical that we, as healthcare professionals, get involved with this and show the importance of patient associations so that we can highlight the need for recognition of these patients. Rare diseases do exist, and we need to disseminate our knowledge, especially to the specialties that will first meet with the patient, mostly within pediatrics. This is because we will only diagnose what we can think of, and we need to offer an excellent treatment plan for these patients as well as excellent follow-up. They deserve it.

 I would like to consider that, in addition to early diagnosis, ideally during neonatal screening, it is crucial to have quick access to drug treatment. We are at an impasse right now because the diagnosis is made—not so early, but still we get there with the support of some pharmaceutical industries— but there is a wait time that has taken longer and longer to start the drug treatment depending on the region within the country. In large cities, it usually takes 3 weeks after the diagnosis to begin treatment, but it should start the very next day. Whereas far away from that, it may take up to 3 months to start the treatment. This is too long of a delay, and the child loses more and more functions. So, we may see a child that, when you prescribed the treatment, they were doing better than the treatment protocol criteria, but when they received treatment, they were already out of the criteria.

 …and the motor neuron cannot wait that long.

 No, the neuron that has died does not return.

 We are living in a unique moment for patients with SMA. There is no doubt—research shows—that neuronal degeneration is intrauterine, so treatment and early diagnosis is key for a better quality of life for these patients. Better quality of life will be achieved when we add drug treatment along with multidisciplinary therapy, so we cannot neglect the importance of the multidisciplinary team. Without them, we cannot have a good response to therapy. Besides that, the importance of the patient associations in supporting these families, as Dr Ortega has said, in offering some equipment before this is offered by the government. So, it is a network that has to be created, a support network for these patients so that we can achieve what we want, which is to reach a better quality of life for them.

The final message that I would like to leave is, first, for physicians who are going to start managing with SMA on a daily basis or eventually diagnose SMA: study the disease, study rare diseases, learn about the natural history of the disease to know the best therapeutical indication and learn the importance of the multidisciplinary team. For the families, the main message is the best therapy is the one you are able to receive the soonest. That is the best therapy and certainly the one that will bring the most benefit because it's at the moment the patient is without therapy that they need to begin treatment, any treatment, to effectively change the natural history of the disease. So, today we talked about various therapeutic options, different mechanisms of action, but the best therapy is the one the patient takes first, and it will be the one that will effectively change the natural history of SMA. For the society as a whole, the message is: we need access to ample treatment, access to comprehensive health policies, and not only access to medication, but access to a multidisciplinary team that is qualified and to ample health care as a whole.

Isso faz toda a diferença ao tentar mudar a história natural da doença; cada segundo conta. Se você faz um diagnóstico precoce, você tem chance de começar um tratamento precoce. Outro ponto, é enfatizar a necessidade de trabalho em equipe. A equipe multidisciplinar precisa se comunicar com transparência para que eles entendam as necessidades do paciente e possam fazer o acompanhamento adequado. Por último, precisamos empoderar familiares, pais e cuidadores para que se filiem a associações de pacientes, e consigam aumentar a visibilidade da doença. Os pacientes estão escondidos porque ninguém pensa na possibilidade de uma doença rara. É muito importante que nós, profissionais de saúde, estejamos envolvidos e mostremos a importância das associações de pacientes para que possamos mudar a necessidade de visibilidade desses pacientes. Doenças raras existem, e precisamos disseminar o conhecimento, principalmente às especialidades que vão encontrar o paciente primeiro, nesse caso, a pediatria. Porque só vamos diagnosticar o que pensamos, e precisamos oferecer tratamento e acompanhamento de excelência para esses pacientes. Eles merecem.



 Eu gostaria de considerar que, além do diagnóstico precoce, idealmente na triagem neonatal, é crucial que tenhamos acesso rápido ao medicamento. Nós estamos em um impasse agora, porque o diagnóstico é feito—não tão cedo, mas conseguimos fazê-lo com o apoio de algumas empresas farmacêuticas—mas existe um tempo que, dependendo da região do país, tem demorado cada vez mais para conseguirmos fazer o tratamento medicamentoso. Nos grandes centros, o tratamento geralmente demora 3 semanas para começar após o diagnóstico, mas deveria começar no dia seguinte. E longe desses grandes centros, pode chegar a até 3 meses para começarmos o tratamento. Esse 

 é muito grande, e a criança vai perdendo cada vez mais e mais funções. Então, às vezes vemos uma criança que, quando você prescreveu o tratamento, ela estava muito melhor que os critérios do protocolo, mas quando ela recebeu o tratamento, ela já estava fora dos critérios.



 …e o neurônio motor não pode esperar esse tempo.

 Nós estamos vivendo um momento único para os pacientes com AME. Não há dúvidas—pesquisas mostram—que a degeneração neuronal tem início no período intrauterino, então tratamento e diagnóstico precoces são chave para uma melhor qualidade de vida desses pacientes. Vamos atingir melhor qualidade de vida quando somarmos o tratamento medicamentoso com a terapia multidisciplinar, então não podemos negligenciar a importância da equipe multidisciplinar. Sem eles, não conseguimos ter uma boa resposta terapêutica. Além disso, a importância das associações de pacientes apoiando essas famílias, como a Dra Ortega comentou antes, em oferecer equipamentos antes que eles sejam fornecidos pelo governo. Então, é uma rede que deve ser criada, uma rede de suporte para esses pacientes, para que a gente chegue no que queremos, que é atingir melhor qualidade de vida para eles.



A mensagem final que eu gostaria de deixar é, primeiro, para os médicos que vão começar a lidar com a AME no dia-a-dia ou, por ventura, diagnosticar a AME: estudem a doença, estudem doenças raras, conheçam melhor a história natural da doença para saber a melhor indicação de cada terapia e aprender a importância da equipe multidisciplinar. Para as famílias, a principal mensagem é: a melhor terapia é a estiver disponível mais rápido. Essa é a melhor terapia e, certamente, a que vai trazer maior benefício, porque é no momento em que o paciente está sem terapia que ele precisa começar algum tratamento para, efetivamente, mudar a história natural da doença. Então, hoje, nós falamos de diversas terapias, diversos mecanismos de ação, mas a melhor terapia é aquela que o paciente tomar primeiro, e será ela que irá mudar a história natural da AME. E, para a sociedade como um todo, a mensagem é: acesso a um tratamento amplo, acesso a políticas de saúde amplas, que não só o acesso ao medicamento, mas acesso a uma equipe multidisciplinar qualificada e à saúde ampla, como um todo.

 In this discussion of rare disease and education, a very important aspect is making people aware of SMA [spinal muscular atrophy] as a diagnosis. Can you speak about this, Dr Kerstenetzky? How is this being addressed in Brazil?

 We have several problems today, and the initial problem is that rare diseases are not included in the curriculum of healthcare schools at the universities. Another point is the lack of students in referral centers, to become familiar with rare diseases. Students at the universities are trained to always look for what is common and not what is rare. But, we can only diagnose what we can think of, and many doctors are under the false impression that they will never encounter a patient with a rare disease. This makes it very difficult to recognize the warning signs. I think we have a debt with the past, we've always learned to only think about rare disease when the investigation of the common diseases had been exhausted. But many times, during the investigation of the common disease, warning signs of a rare disease can be perceived, and we can investigate in parallel and reduce the great distance between initial symptoms and diagnosis. So, it is necessary to change this approach so that the topic of rare diseases is included in the universities. If not, we are going to continue with that multitude of diagnoses. How can a healthcare professional make a diagnosis of a disease that they have never heard of? It is very difficult when, after years, patients get the diagnosis. They question the doctor who made the diagnosis, ‘Why didn't anyone think of this possibility before? I have been looking for a diagnosis for 7, 8, 9, 10 years, I have been to several specialties, seen several professionals, and no one thought of the possibility of a rare disease.’ So, we must change this context. Once again, I repeat, to emphasize the need for awareness that the rarity of a disease does not exclude its existence. A rare disease may be right next to us, we just need to stop and think. You can only diagnose what you can think of. So, if you don't look for it, you won't find it. If you can’t find it, you won’t have the ability to offer a treatment for these patients.

 Nós temos vários problemas, e o problema inicial é que doenças raras não são inseridas nos currículos das escolas de saúde nas universidades. Outro ponto é a falta de estudantes nos centros de referência para que eles estejam familiarizados com as doenças raras. Os estudantes foram treinados nas universidades a sempre procurer o que é comum, e nunca o raro. Mas nós só vamos diagnosticar aquilo sobre o que conseguirmos pensar, e muitos médicos tem a falsa impressão de que nunca irão encontrar um pacience com uma doença rara. Isso dificulta muito o reconhecimento dos sinais de alerta. Eu acho que temos uma dívida com o passado, nós sempre aprendemos que só poderíamos pensar na doença rara quando se esgotasse a investigação da doença comum. Mas muitas vezes, durante a investigação de uma doença comum, alguns sinais de alerta para uma doença rara podem ser percebidos, e nós podemos investigar em paralelo e diminuir essa grande distância entre os sintomas iniciais e o diagnóstico. Então, é necessário que mudemos essa visão para que o tema ‘doença rara’ seja inserido nas universidades. Senão, vamos continuar com aquela odisseia de diagnóstico. Como um profissional de saúde fará um diagnóstico de uma doença que ele nunca ouviu falar? É muito difícil quando, após anos, pacientes tem o diagnóstico. Eles questionam o médico que os diagnosticou, ‘Por que ninguém pensou nessa possibilidade antes? Eu estou há 7, 8, 9, 10 anos buscando um diagnóstico, já fui a vários especialistas, consultei com diversos profissionais, e ninguém pensou na possibilidade de uma doença rara.’ Então, precisamos mudar esse contexto. Mais uma vez, eu repito, para enfatizar a necessidade de se conscientizar que a raridade de uma doença não exclui a sua existência. A doença rara pode estar ao nosso lado, nós só precisamos parar e pensar. Você só vai diagnosticar o que você pensar. Então, se você não procurar, não irá encontrar. Se não encontrar, não conseguirá oferecer tratamento a esses pacientes.

Rare Diseases Awareness and Educational Needs / Conscientização sobre doenças raras e necessidades educacionais

 Let’s broaden in terms of our advice for those who are listening whether they are physicians or other advanced care providers on the team. What resources are available for them in Brazil in caring for SMA [spinal muscular atrophy]? What advice do you have for SMA care teams in terms of optimizing care?

 For the doctors that are going to follow patients with SMA, it is important that they consider several areas including knowing the disease, the natural history of the disease, the medications that are being used which modify the natural history of the disease, as well as the adverse events. From there on they can start following up with these patients. Today, in Brazil, because of the size of our country, there are professionals who are starting to treat patients with SMA, but who don’t have much experience with the disease. So, the recommendation for them is that they should be well-versed with the disease, with the natural history of the disease, medications and the multidisciplinary care that has to be offered to these patients and, in this sense, build a team. Another very important aspect for physicians: for SMA and other neuromuscular diseases, we don't treat them alone; we treat with a multidisciplinary team that is well aligned, knows the disease and that is prepared to assist these patients. Continuing education programs have been increasingly offered in Brazil, and this has brought more information and awareness to people. And, as Dr Kerstenetzky has said, Brazil is very large. We need more referral centers, better dissemination throughout our geographic territory.

As Dr Giannetti highlighted, there is an exponential growth of physicians interested in treating SMA in Brazil. But we know that it is not enough to just learn about medications and that they are indicated for patients in age group A, B, or Z, but they should learn about the natural history of the disease, how far the medication can interfere in the natural history, what it will change in the patient’s journey in order to share the correct expectations with the family. Furthermore, we must have a multidisciplinary team, not only the medication. What we are still miss in Brazil is that physicians are aware that the medication alone is not sufficient; we need an engaged multidisciplinary team.

 I think this recognition should come not only from the physicians, but from the Ministry of Health itself. When they develop the Clinical Protocols and Therapeutic Guidelines, they must understand that it is necessary to offer medication, but also offer all the basic care for the patient. We don't see this happening; we end up having access to medication but not to rehabilitation, and we know that they should go together.

 In regard to the doctors, I think that Dr Kerstenetzky has a lot of experience in medical education for rare diseases, right?

 Absolutely. This is vital, because doctors will only diagnose what they can think of, and one of the main points is to make them aware that the rarity of a disease does not exclude its existence. So, medical education is very important, and when I say continuing medical education it should be extended to other health care professionals, because they are involved in the multidisciplinary and clinical follow-up of SMA. When we share our knowledge, we extend the life expectancy of patients. Knowledge dissemination is very important so that we can reach all professionals in the multidisciplinary team.

 In Brazil, we have patient associations playing this role of continuing education, training professionals such as physical therapists and speech therapists in understanding the disease and basic patient care.

 It is very important, Dr Mendonça, to remind patients and families that ‘when alone, they may be rare, but as a group, they are stronger,’ so they have better exposure. It is important that patients and families join these associations because they support new achievements and greater visibility for the disease; not only by fully integrating patients into society, but also by raising awareness from the health care point of view for these patients. So, it is very important that we remember the importance of having our patients affiliated to these associations.

 The lack of adequate support from the government is a gap in care for these patients. I think Dr Ortega can speak a little bit about her experience.

 The government, as Dr Giannetti said, should provide the support, but unfortunately it cannot provide everything. This may be due to some difficulty in identifying where the patients are located and providing training for therapists and doctors in these regions. Because patients come from everywhere—from the countryside, small towns—they are not only in the large cities. Therefore, at this moment in time, the patients' associations are doing what would be the Ministry of Health’s function. They are providing adequate training, and at many times they are securing the equipment that these children need. They rent the mechanical ventilators until the government can make one available for that child. At many times, they train therapists by hiring a therapist from a large center to train local therapists 

, to detail how the best rehabilitation and therapy should be for that patient. In another front, these associations have also been helping parents, caregivers and the families of the children diagnosed with SMA. So, they provide support with the psychological and emotional issues, helping families to feel accepted and see that they are not rare, even though it is a rare disease, that together they are stronger. In addition, they help in acknowledging all the problems, difficulties, and changes that the disease can cause. Because, many times, when they hear from the doctor that the child may have respiratory issues, dysphagia, and that a support with an enteral tube or gastrostomy will be necessary, the families get scared, and they think this is a big intervention. When someone who has already gone through the same situation comes along, who has a child with the same rare disease and that, when a gastrostomy was placed, the child stopped having respiratory infections, bronchial complications, and improved weight gain, the family accepts it more easily because they feel recognized by the other. So, they have a very important role in our society, in supporting these patients and even to have a voice with the government, to fight with the Ministry of Health—to fight in the good sense, as in request—that they look at rare diseases and provide adequate support.

 Dr Ortega was talking about the importance of family associations in helping with the whole treatment, not only in the support care but also accepting medical needs that they are resistant to at the beginning.

 I think everybody is different about how much they can connect with others when they are processing something as significant as a diagnosis like this. But it does help to connect with others who are also living with SMA. This can help you gain valuable insight as you make your own care decisions for managing the disease. 

 Vamos ampliar nossos conselhos para aqueles que estão ouvindo, sejam eles médicos ou outros profissionais de saúde na equipe. Quais recursos estão disponíveis no Brasil para o cuidado da AME [atrofia muscular espinhal]? Qual conselho vocês tem para equipes que lidam com AME em termos de otimização de cuidados?

 Para médicos que vão acompanhar pacientes com AME, é importante que eles conheçam a doença, a história natural da doença, as medicações que estão sendo usadas que modificam a história natural da doença, e também os efeitos adversos. A partir daí, eles podem iniciar o acompanhamento desses pacientes. Hoje, no Brasil, por causa do tamanho do nosso país, existem profissionais começando a tratar pacientes com AME, mas que não tem muita experiência com a doença. Então, a recomendação para eles é que estudem bastante sobre a doença, a história natural da doença, medicações e o cuidado multidisciplinar que deve ser oferecido a esses pacientes e, nesse sentido, construir uma equipe. Outro aspecto muito importante: AME e outras doenças neuromusculares, nós não as tratamos sozinhos; nós as tratamos com um time multidisciplinar que esteja bem alinhado, conheça a doença e esteja preparado para assistir esses pacientes. Cursos de educação continuada tem sido, cada vez mais, oferecidos no Brasil, e isso tem trazido mais informação às pessoas. E, como o DrKerstenetzky falou, o Brasil é muito grande. Precimos de mais centros de referência, de maior capilaridade no nosso território geográfico.



Como a Dra Giannetti destacou, temos visto um crescimento exponencial de médicos interessados em tratar AME no Brasil. Mas nós sabemos que não é suficiente aprender apenas sobre o medicamento ou que ele é indicado para pacientes da faixa etária A, B, ou Z. Os médicos devem aprender sobre a história natural da doença, o quanto a medicação pode interferir na história natural da doença, o que pode mudar na trajetória daquele paciente para compartilhar as expectativas corretas com a família. Além disso, ter uma equipe multidisciplinar, não é somente o medicamento. O que ainda falta no Brasil é que o médico esteja consciente de que não é só o medicamento, precisa-se de uma equipe multidisciplinar engajada.

 Eu acho que esse reconhecimento não deve partir somente do médico, mas do próprio Ministério da Saúde. Quando eles desenvolvem o PCDT [Protocolos Clínicos e Diretrizes Terapêuticas], eles precisam entender que é necessário oferecer medicação, mas também oferecer cuidados básicos para aquele paciente. Nós não vemos isso acontecer; acabamos tendo acesso à medicação mas não à reabilitação, e nós sabemos que elas devem caminhar juntas.

Em relação aos médicos, eu acho que o Dr Kerstenetzky tem bastante experiência em educação médica para doenças raras, certo?



 Sem dúvida. Isso é vital, porque os médicos só irão diagnosticar o que eles pensam, e um dos principais pontos é conscientizá-los que a raridade de uma doença não exclui a sua existência. Então, educação médica é muito importante, e quando eu falo educação médica continuada ela deve ser estendida a outros profissionais de saúde, porque eles estão envolvidos no acompanhamento clínico e multidisciplinar da AME. Quando dividimos nosso conhecimento, multiplicamos a esperança de vida dos nossos pacientes. Portanto, disseminação do conhecimento é muito importante para que possamos alcançar mais profissionais nas equipes multidisciplinares.

 No Brasil, nós temos associações de pacientes fazendo esse papel na educação continuada, treinando profissionais como fisioterapeutas e fonoaudiólogos no entendimento da doença e cuidados básicos com o paciente.

 E é muito importante, Dr Mendonça, lembrar os pacientes e familiares que sozinhos, eles podem ser raros, mas em conjunto, são muito mais fortes, e isso cria maior visibilidade. É importante que eles se integrem às associações de pacientes e familiares porque isso apoia novas conquistas e traz luz à doença; não somente ao integrar os pacientes à sociedade, mas também ao trazer visibilidade do ponto de vista de saúde para esses pacientes. É importante que a gente lembre da importância de eles estarem afiliados a essas associações.



 A falta de suporte adequado vinda do governo é uma lacuna no cuidado com esses pacientes. Eu acho que a Dra Ortega pode falar um pouco sobre a experiência dela...

 O governo, como a Dra Giannetti disse, deveria fornecer o suporte, mas infelizmente não consegue suprir tudo. Talvez por uma dificuldade em identificar onde estão os pacientes e oferecer treinamento para terapeutas e medicos nessas regiões. Porque os pacientes vem de todos os lugares—do interior, de cidades pequenas—eles não estão só nos grandes centro. Então, nesse momento, as associações de pacientes vem exercendo a função que seria do Ministério da Saúde. Eles estão oferecendo treinamento adequado, e muitas vezes conseguindo os equipamentos que essas crianças precisam. Eles alugam respiradores até que o governo consiga disponibilizar um para aquela criança. Muitas vezes, eles treinam os terapeutas da cidade de origem ao contratar um terapeuta de um grande centro para treinar os terapeutas locais, 

, para detalhar como deve ser a melhor reabilitação e terapia para aquele paciente. Em outro sentido, as associações também tem ajudado pais, cuidadores e familiares das crianças diagnosticadas com AME. Eles dão suporte nas questões psicológica e emocional a essas famílias, ajudando-as a se sentirem acolhidas e verem que não são raras, apesar de ser uma doença rara, e que juntos, tem mais força. Inclusive, reforçando todos os problemas, dificuldades e alterações trazidas pela doença. Porque, muitas vezes, quando eles ouvem do médico que a criança pode ter dificuldade respiratória, disfagia, e que irá precisar de suporte com tubo enteral ou gastrostomia, as famílias se assustam e acham que é uma grande intervenção. Quando vem alguém que já passou pela mesma situação, com um filho com a mesma doença rara e que, quando fez a gastrostomia, parou de ter infecções respiratórias, complicações com broncoaspirações, melhorou o ganho de peso, a família aceita com mais facilidade porque se sente reconhecida no outro. Então, elas tem um papel muito importante na sociedade no apoio desses pacientes, e até mesmo para ter uma voz junto ao governo, para brigar com o Ministério da Saúde—brigar no bom sentido, como solicitar—para que o Ministério olhe para as doenças raras e dê o suporte adequado a elas.



 Dr Ortega estava falando da importância das associações de pacientes e familiares no apoio ao tratamento, não somente nos cuidados mas também em aceitar procedimentos médicos aos quais as famílias são muito resistentes no início.



Eu acho que todos são diferentes no sentido de se conectar com outras pessoas enquanto estão processando algo tão significativo quanto um diagnóstico de AME. Mas, certamente, estar conectado com outros que também estão vivendo com AME pode ajudar muito. Isso pode te dar acesso a informações importantes para que você possa fazer suas próprias decisões sobre cuidado e manejo da doença.

 Another area that we need to be careful about justice and access is clinical trial participation, and how people can get access to research as opposed to what is available in the clinic. I am curious about your Brazilian experience and how it goes from center to center?

 Clinical trials in Brazil are restricted to large centers. We have some challenges when starting a clinical trial. First, this depends on the patient profiles that have to be recruited, how fast we can diagnose—we have to quickly analyze if this patient fits the inclusion criteria. Since we still have some difficulty to make a diagnosis, and sometimes it takes a while, we can lose patient eligibility to enter that clinical trial. The second biggest challenge is really social issues. This is an aspect that, when we run a clinical trial, we have to consider when talking to the family. What are the resources available to that family so they can combine therapy with the rehabilitation work that we just talked about. Brazil is a large country, and we have few centers that conduct clinical trials. We take patients from other states who come to receive the medication and go back to their state of origin. So, it is important to have the logistics organized, to know if the family is going to adhere to the study and if they can carry out an adequate rehabilitation because if not, this can bring differences in the study results. What is the biggest challenge that you see in the center where you work, Dr Mendonça?

 The experience of working on a clinical trial is very interesting. You can see how to evaluate the efficacy of a new drug, but it is a challenge in Brazil. Because the country is so large, there is great heterogeneity in basic patient care, rehabilitation, and especially care when an adverse event arises. So, in practice, besides the difficulty of having the patient travel—imagine a patient with SMA type 1 that has to travel from the state of Minas Gerais to São Paulo. So, when they have an adverse event, which is when you need to monitor them the closest, they are far from you. Then, accessing the patient’s data on that adverse event becomes very difficult and complicated to report at the end of the study. This limits us a lot, for example, to place patients from the countryside in clinical trials; many times, we end up not placing them…

 …for social reasons. This is a limitation that I see more and more. Sometimes we have eligible patients, but we must think about the social aspects, whether they are going to adhere to the study, and if they are going to have a good rehabilitation with the support they need when they are away from the referral center.

 Dr Kerstenetzky has a lot of experience with RARUS in Recife, on having a center for rare disease, the difference it makes for families, and how much social aspects can interfere, right?

 Absolutely, Dr Mendonça. The greatest challenge is that Brazil is a continental-sized country. There are almost 8 and a half million square kilometers and a population of 218 million inhabitants—many times the access to these reference centers is very difficult. In Brazil, there are only 18 referral centers for rare diseases recognized by the Ministry of Health. The National Policy of Comprehensive Care for Patients with Rare Diseases was created in 2014 with the goal of reducing morbidity, mortality and offering better quality of life for patients. But many patients are far from the referral centers, so their access is very difficult. Not only to the clinical follow-up with the multidisciplinary team and laboratory tests, but also access to treatment. We must have more referral centers in different parts of Brazil. For example, the northern region of Brazil does not have any referral center officially recognized by the Ministry of Health, so it is very difficult to follow through with patients. At our center, RARUS, we communicate with the doctors in the inner cities to orient them about how difficult it may be for the patient to go to our center, so they can monitor the laboratory exams and constantly communicate with us, so that we can manage a complication that may arise.

 We were talking about our difficulties in Brazil with very few referral centers for rare disease, neuromuscular diseases, and so forth. So, we face a problem that precedes inclusion of patients in clinical trials. Many times, we are not able to include a patient in a clinical trial because he is too far from us. It is not actually the problem of going back and forth from the center to monitor the study, but the primary attention that the patient does not have at their site of origin.

 Outra área sobre qual precisamos ser cuidadosos e justos é no acesso à participação em estudos clínicos, e como as pessoas conseguem acessar as pesquisas e não somente o que está disponível na clínica. Estou curioso para saber sobre a sua experiência no Brasil e como funciona de um centro para outro?



 Fazer estudos clínicos no Brasil está restrito aos grandes centros. Nós temos alguns desafios quando começamos um estudo clínico. Primeiro, dependendo do perfil de paciente que deve ser incluído, a rapidez do diagnóstico—temos que avaliar, rapidamente, se o paciente está nos critérios de inclusão. Como ainda temos dificuldade em fazer um diagnóstico, e algumas vezes isso leva tempo, podemos perder a eligibilidade daquele paciente para entrar no estudo clínico. O segundo maior desafio que temos são questões sociais. Esse é um aspecto que, quando fazemos um estudo clínico, temos que considerar ao falarmos com a família. Quais são os recursos dessa família para que ela possa combinar a terapia com o trabalho de reabilitação que acabamos de conversar. O Brasil é muito grande, e temos poucos centros que fazem estudos clínicos. Nós recebemos pacientes de outros estados para receberem a medicação e voltarem para o seu estado de origem. Então, é importante termos essa logística organizada, para saber se a família terá uma adesão ao estudo e se eles podem realizar uma reabilitação adequada, porque se não, isso pode fazer diferença nos resultados dos estudos. Qual o maior desafio que você vê no centro onde você trabalha, Dr Mendonça?



 A experiência de trabalhar num estudo clínico é muito interessante. Você consegue avaliar a eficácia de uma nova droga, mas no Brasil é um desafio. Devido ao país ser muito grande, há grande heterogeneidade na atenção básica do paciente, na reabilitação e, principalmente, no cuidado que o paciente terá quando um evento adverso acontecer. Então, na prática, além da dificuldade no deslocamento—imagine um paciente com AME tipo 1 que precisa viajar do estado de Minas Gerais até São Paulo. Então, quando eles tem um evento adverso, que é quando nós precisamos monitorá-los mais de perto, eles estão longe. Nesse caso, acessar os dados do paciente sobre aquele evento adverso se torna muito difícil e complicado de reportar no final do estudo. Isso nos limita muito, por exemplo, para incluirmos um paciente do interior nos estudos clínicos; muitas vezes, nós acabamos não os incluindo...



 …por questões sociais. Essa é uma limitação que eu vejo cada vez mais. Às vezes, temos pacientes elegíveis, mas precisamos pensar sobre os aspectos sociais, se eles realmente terão boa adesão, reabilitação, bom suporte quando eles não estiverem dentro do centro de referência.

 Dr Kerstenetzky tem muita experiência com o RARUS em Recife, na questão de ter um centro de referência em doenças raras, como faz diferença para as famílias, e o quanto a questão social interefere, certo?



 Sem dúvida, Dr Mendonça. A grande dificuldade é que o Brasil é um país de dimensões continentais. São aproximadamente 8.5 milhões de quilômetros quadrados e uma população de 218 milhões de habitantes—muitas vezes, o acesso a centros de referência é muito diíficil. No Brasil, existem apenas 18 centros de referência para doenças raras reconhecidos pelo Ministério da Saúde. A Política de Atenção Integral aos Pacientes com Doenças Raras foi criada em 2014 com o objetivo de diminuir mortalidade, morbidade e oferecer melhor qualidade de vida aos pacientes. Mas muitos pacientes estão distantes dos centros de referência, então o acesso é muito difícil. Não somente para o acompanhamento clínico com a equipe multidisciplinar e os testes laboratoriais, mas também o acesso ao tratamento. Precisamos de mais centros de referência em diferentes partes do Brasil. Por exemplo, a região norte do Brasil não tem nenhum centro de referência oficialmente reconhecido pelo Ministério da Saúde, então fica muito difícil acompanhar esses pacientes. No nosso centro, RARUS, temos uma interface com os médicos do interior para orientá-los sobre as dificuldades que os pacientes tem em ir ao nosso centro, para que eles possam monitorar exames laboratoriais e estar em constante comunicação conosco, para que possamos lidar com qualquer complicação que possa aparecer.



 Estávamos falando da nossa dificuldade no Brasil com tão poucos centros de referência para doenças raras, doenças neuromusculares e outras. Temos um problema que precede a inclusão de pacientes em estudos clínicos. Muitas vezes, não conseguimos incluir um paciente em um estudo clínico porque ele está muito distante de nós. De fato, não é somente a questão de ir e voltar ao centro para monitorar o estudo, mas a atenção primária a qual o paciente não tem acesso no seu local de origem.
